I have posted the initial paperwork here so that you may fill it out and bring it to your first appointment. Please review all pages as they contain important information regarding my business policies and therapy services as well as your rights and responsibilities.

For both phone messages and
e-mail, I will usually get back to you within 24 hours unless it is Friday afternoon or a weekend/holiday. During these times I will return your call or e-mail on the next business day. If you don’t hear back from me in 36 hours, please try to call me again.
Rescheduling and Cancellation
I kindly request that you give me a 24-48 hours notice in case you need to cancel or reschedule your appointment.
